The mechanism of actions of various anticoagulants in a nutshell:
1. Aspirin: Irreversibly inhibits COX enzyme (antiplatelet)
2. Warfarin: Inhibits coagulation factors II, VIII, IX, X, Protein C and Protein S
3. Conventional heparin: Inhibition of factor Xa through inhibition of anti-thrombin III (AT-III) and also inhibition of factors II, IX, XI, XII
4. Low molecular weight heparin (LMWH heparin): Inhibition of factor Xa through AT-III and also inhibition of factor II
5. Fondaparinux: Inhibition of factor Xa through AT-III
6. Rivaroxaban: Direct Xa inhibitor
7. Hirudin: Direct IIa inhibitor
